The Nurse Education Program is a state approved program-of-study and prepares students to successfully pass examinations for licensure in the field of health care. The program's quality curriculum and training meet current and future needs of an ever changing health care delivery system. The Nurse Education Office is located at 1452 David E. Need a Transcript?

1. Complete and print a transcript request (Form). Request forms are also available in the Nursing Office.


2. The request must be signed and submitted by the person requesting the transcript.


3. Hand deliver or mail the transcript request to the CAE bookstore at:

4. Cost per official transcript is $3.00 payable to CAE by check or money order.


5. Please allow 7 to 14 days for processing of transcripts.
